I’ve studied Japanese for too long; I don’t see the images you describe at all. Instead my mind breaks the kanji down into their component radicals (which are like mini kanji within kanji).
*chuckle* Okay, here. For the girl on the left, the wide horizontal line is the brim of her hat. the sweeps left and right are her shoulders. She’s looking to the right.
For the ouch! on the right, the right-most separate unit looks exactly like a toaster with smoke coming out. you have the profile of an “ouchy” face on the left (kinda like the poison control face), where the dot by itself is the eye as it looks toward the toaster. the top-most line with the dot on top is obviously (hee) a hand.

:)
I see the toaster! I see the toaster!
wait…
I DO see the girl in the hat! YAY
I see it all!
I see the toaster, but for the one on the left, I am thinking it looks a lot more like two people hugging under the sakura blossoms.
Is it some kind of illusion? Do I have to look through the tattoo?
;)
To what, blood vessels? :)
